Survey: How Do You Feel About Returning to In-Person Worship?

Dear City Church,

Recently, we formed a Phased Re-Opening Task Force to help guide City Church through a process of resuming gathered worship and restarting other ministry programs suspended because of restrictions related to the pandemic. The Task Force has put together a short survey to help us better understand the needs and desires of the congregation. The questions are intentionally broad and open-ended to facilitate a wide variety of feedback. We’d be grateful if you took a few moments to complete it.

We're hoping to gather all responses by Wednesday, May 27th—that's one week from today. This will allow our Re-Opening Task Force time to review responses thoroughly.
